- We were asked to continue project reviews and Contractor Proposals for LV Distribution, focusing on origin, which was out of scope - the order was not for design albeit design activity took place. The metering on the revised LV schematic was based on the previous contractor’s design - no longer involved. The previous did not allow for separate metering of Lighting.
- The meter to IT DB has been omitted or missed, however, the previous contractor went under and there was no one to ask
- No allowance has been made by the consultant nor the contractor for separate lighting meters - no on questioned it before
- Bit of problem with ownership of the responsibility for the design - continued requests for comments on someone’s proposals. Without clearly defined and expandable scope
- Design was late, the contractor on-site building the project while the detailed technical design was in progress, with a significant number of known significant and ongoing issues
- Dig deeper: Use techniques like the "Five Whys" to dig deeper into each immediate cause. Ask "why" repeatedly to trace the causal chain until you reach the root cause.
